Conszela Sept is from Baton Rouge, Louisiana that’s a licensed cosmetology professional for 15+ years and a licensed cosmetology instructor. She specializes in healthy hair of all types, haircuts as well as a weave specialist. Her ultimate goal is to be a light in the beauty industry by mentoring, motivating, inspiring and educating current and future professionals. Salon Policy
Upon booking a appointment with { ConSept Hair Designs] I agree and automatically sign my signature and accept full responsibility to these terms. I verify that I understand and agree to the following terms and conditions for receiving a chemical service at { ConSept Hair Designs}
- I understand that chemical treatments have different effects on different hair types and colors. I agree not to hold the salon and the hair technician harmless in the event of unexpected or undesired results. I understand that any further alterations or corrections will be provided at my own expense.
- I have had the process and possible results adequately explained to me. I have been given the opportunity to make requests and ask questions.
- I understand that previous treatments and chemical services can continue to have an effect on my hair. It has been recommended to me that I fully disclose my previous treatments to my stylist prior to the treatment.
- I understand that the chemicals may have unexpected and undesired effects on the metals in my jewelry. I verify that I have been given the opportunity to remove my jewelry, as well as a safe place to keep it for the duration of the treatment.
- I understand that my stylist is a certified technician who will try his or her best to create my desired results. I understand that the best way to assist my technician is to be clear about my requirements. I knowingly and willingly consent to having hair and salon service(s) during the COVID-19 pandemic. I understand and accept this statement.
- To prevent the spread of contagious viruses and to help protect each other, I understand that i will have to follow the salon's strict guidelines I understand and accept this statement.
- I understand that air travel significantly increases my risk of contracting and transmitting the COVID-19 virus. I know that the CDC, OSHA, and North Dakota state board of cosmetology recommend social distancing of at least 6 feet. I understand and accept this statement.
- I understand that due to the frequency of visits of other clients, the characteristics of the virus, and the characteristics of hair services, that I have elevated the risk of contracting the virus by merely being in the salon company. I understand and accept this statement.
- I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it, and who does not give the current limits in virus testing. I understand and accept this statement.
- I verify that I have not traveled outside the United States In the past 14 days to countries that have been affected by COVID-19 *
- I confirm that I have or have not traveled domestically within the United States by commercial airline, bus or train within the past 14 days
- In-salon Temperature Policy I’m willing to take a temperature check during my visit to the salon before the services are started, and I agree not to come to the salon with the following symptoms of COVID-19 listed below: Fever- Temperature Shortness of breath Loss of sense of taste or smell Dry cough Runny nose Sore throat
- I understand, read, and completed this questionnaire truthfully. I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I understand that this document is to provide the best possible guest experience during my salon visit.
